A 76 years old man with persistent atrial fibrillation. The patient was hospitalized to perform electric cardioversion. He was previously failed pharmacologic cardioversion by propafenone. Transesophageal echocardiography (TEE) was performed before electric cardioversion. In TEE, non-mobile linear structures were observed attached from the interatrial septum in the left atrium and there was no flow limitation in the color Doppler study. There were no significant regurgitation or stenosis of the other valves. We considered this linear structure as congenital left atrial band (CLAB). After electric cardioversion, it was converted to sinus rhythm. RFCA will be implemented.
